Reconstruction options following subtotal gastrectomy include Billroth II (B2) and Roux-en-y. Both are associated with similar overall perioperative morbidity and mortality; bile gastritis and marginal ulcer, however, occur more often after B2. Conversion of B2 to Roux-en-y is an option for patients with marginal ulcer that fail medical therapy. Robotics has been described in bariatric surgery and primary resection of gastric cancer, but little has been published on its use in revisional surgery in gastric cancer patients.
We report a case of a 61-year-old female with history of Stage 2a gastric cancer who had undergone a subtotal gastrectomy and Billroth II in 2014. She presented to clinic one year later with a marginal ulcer at the gastrojejunostomy. We planned for a totally robotic revision of her Billroth II to Roux-en-y gastrojejunostomy with hiatal hernia repair.
