We present a video case presentation detailing two different surgical approaches to the management of intrathoracic gastric sleeve migration after sleeve gastrectomy. The first patient underwent laparoscopic hiatal hernia repair with cruroplasty, while the second patient underwent hiatal hernia repair with cruroplasty and revision of gastric sleeve to Roux-en-Y gastric bypass (RYGB). Reduction of the migrated stomach with cruroplasty should be performed in symptomatic patients with intrathoracic sleeve migration. Whether revision of the sleeve with resection of redundant stomach or conversion to RYGB should be performed is still debated.
