A 68 years old female presents with liver metastasis 5 years after a distal laparoscopic pancreatectomy for a 1 cm adenocarcinoma. The main and the satelite lesions are located in segment 4A.
The supra hepatic area is dissected. A tourniquet is applied around the liver pedicule. The left glissonian is directly stapled. The hepatotomy line is demarcated. The hepatotomy is done with the hook, bipolar, ultrasonic aspirator and staplers. The middle hepatic vein is a landmark for the resection. It’s dissected and preserved . The lesions are resected with adequate margins. Hemostasis is double checked. Gallbladder is removed. The area is irrigated and a sealant is applied. A drain is left in place.
