This is a video of a novel approach to gender-affirming surgery, laparoscopic colovaginoplasty through the harvest of the right colon to function as the neo-vaginal graft. Gender-affirming surgery offers major psychological and quality of life improvements for transgender patients. For male-to-female transgender patients, intestinal vaginoplasty offers functional benefits such as natural mucus production and greater canal depth without risk of contraction. Though the sigmoid colon segment is most commonly used in colovaginoplasty, we seek to introduce the use of the right colon as a neovaginal graft. The right colon offers more facile reach of the ileocolic vascular pedicle, creation of anastomosis away from neovagina in the pelvis, and creation of a less risky anastomosis in the form of an ileo-transverse colon anastomosis.
