Here we present a case of a 35 year old male who underwent an endoscopically assisted laparoscopic truncal vagotomy and distal gastrectomy with gastrojejunal anastomosis (BII). The utilization of the endoscope in this case ensured that there was no injury to major hepatic structures in an otherwise difficult operative field. Additionally, the edited version highlights the key steps to an operation not commonly performed.
