Endoscopic closure of luminal defects proves difficult when working space is tight. Instances such as these may benefit from the use of more than one method of closure to achieve the desired result. This video demonstrates both a complication of working within a tight space, wherein the Overstitch device becomes entangled in the suture line as well as dislodgement of the cap, by which it was liberated. Additionally, the “Triple O” closure method is explained, whereby an OTSC clip is placed over an Overstitch endoscopic suture closure.
