A case of a 68-yeay-old male with history of familial adenomatous polyposis who has a dysplastic lesion around the angle of His that is not amenable for endoscopic therapies despite several attempts of ablation and endoscopic mucosal resections. The patient was offered gastrectomy. A laparoscopic transgastric approach was used to resect the lesion. After the closure of the gastrotomies an air water test was performed to confirm that there was no leak.
