This is a 50 year-old female with a history of severe peptic ulcer disease, including a history of a perforated ulcer near the pylorus who has developed gastric outlet obstruction secondary to this process.
She presented for laparoscopic highly selective vagotomy combined with pyloroplasty.
The objective of the highly selective vagotomy and pyloroplasty is to not only denervate acid producing parietal cells while preserving gastric motility but to also alleviate the obstruction from peptic ulcer stricture and should be used in selective patients.
