Transanal total mesorectal excision (TaTME) is a novel method for rectal resection. This technical advance facilitates rectal resection with minimally invasive techniques and overcomes the challenges associated with low rectal stapling. We present a case of TaTME assisted proctocolectomy and pelvic pouch formation in a patient with early upper rectal cancer in a patient with known ulcerative colitis and surveillance demonstrated high grade dysplasia.
