Laparoscopic Roux-en- Y gastric bypass (LRYGB) is one of the most effective bariatric procedures and internal hernia is a known complication of this procedure. Rates in the literature range from 0.9 to 9%. Internal hernias present as intermittent left upper quadrant pain or symptomatic bowel obstructions. Chylous ascites / chyloperitoneum with bowel obstruction is a relatively rare event in the literature with a total of 22 cases from bowel obstruction reported thus far. We report a case of jejunojejunostomy inversion and chylous ascites secondary to an internal hernia after LRYGB.
