Remediation of multi-fundoplication failure is one of the most challenging tasks faced by foregut surgeons in terms of selecting the most appropriate operation as well as the technical difficulty of dissecting out a scarred-in hiatus. This video depicts the case of a young patient who had undergone three prior laparoscopic hiatal hernia repairs with Nissen fundoplication, the second of which had included placement of biologic mesh. All failed due to wrap herniation resulting in dysphagia and recurrent reflux symptoms. In this video we illustrate our algorithmic approach to remediation of the multi-fundoplication failure patient and we highlight some of the key maneuvers we feel critical in achieving a durable and optimal result.
