Superior mesenteric artery syndrome is rare, but has been reported in the bariatric surgery population in case reports. We describe a patient who developed SMA syndrome following rapid weight loss from sleeve gastrectomy. The workup and management options are discussed, with video documentation of our surgical approach. We show the various steps of laparoscopic duodenojejunostomy with endoscopic assistance. The patient had an excellent clinical outcome and significant relief of her symptoms.
