Novel Technique for Direct Access Transgastric ERCP (DATE) Using a Single Site Platform

We report our technique for direct access transgastric ERCP (D.A.T.E.) using a single-site platform. The procedure is exemplified by this case of a 54 year old female with a history of Roux-en-Y gastric bypass who presented with persistent right upper quadrant pain after recent laparoscopic cholecystectomy. MRCP revealed choledocholithiasis, but double-balloon ERCP failed. The patient underwent D.A.T.E. with a modified single-access device, and her choledocholithiasis was successfully cleared. This video illustrates our technique.
