This is a case presentation of the complex endoscopic management of a chronic gastric fistula. The real-time interpretation of fluoroscopic images directly impacted intraoperative decision making in this case, highlighting the importance of liberal use of fluoroscopy during these procedures as a diagnostic adjunct to the endoscopic view. Without fluoroscopy, the patient would have had undiagnosed areas of additional gastric leaks and a much more prolonged course. This presentation also highlights the management of chronic fistulae using endoscopic tools while adhering to basic surgical principles.
