This is an operation video of puretaTME completed in 2014. The patient was a middle-aged woman and diagnosed as low rectal cancer. We made purse-bag suture from the lower edge of the tumor about 1 cm, then followed by free perirectal mesenteric space, until the peritoneal reflex was opened, and continued to free to the root of the mesenteric vessels and ligation, pulled out the specimen through the anus and anastomosed the end-to-end anastomosis stapler, and left drainage tube around the anus. The operative time was about 2 hours and 30 minutes, and the bleeding during operation was about 50ml.
DISCUSSION: For some female patients, complete transanal total mesorectal excision is feasible under suitable conditions, which is conducive to postoperative recovery. At present, 26 cases of puretaTME have been completed in our unit, which confirms the safety and feasibility of the operation.
