This video presentation will review a laparoscopic spleen preserving distal pancreatectomy via a unique approach. We present the video using cognitive task analysis to break down the complex procedure into 9 specific modular components in order to simplify each task. This will aid surgeons adopting the technique in their practice, fellows understanding of the procedure, and myriad learners as they follow the step wise approach to saving the spleen during distal pancreatectomy for benign pathology. Slides for each module are incorporated that summarize each step and review our tips and tricks. The 9 modules include: entering the lesser sac, taking down the short and posterior gastrics, taking down the splenic flexure, the superior mobilization of the spleen, identifying pancreatic pathology, the retropancreatic dissection, isolating the splenic vein and artery, pancreatic transection and removing the specimen.
