In this video, we demonstrated laparoscopic reversal of Single Anastomosis Gastric Bypass (Mini-Gastric Bypass / Omega Loop Gastric Bypass) to sleeve gastrectomy in patient who was TPN dependent due to severe malnutrition and dysphagia. Patient had a history of undergoing sleeve gastrectomy followed by revisional procedure to single anastomosis gastric bypass (with gastroileostomy anastomosis), as well as emergency Graham patch repair due to perforated marginal ulcer. A revision to the original sleeve gastrectomy was performed. The gastroileostomy was removed, and the gastrogastrostomy anastomosiswas created on the proximal and distal portions of the sleeve in order to restore the gastric continuity. Operative time was 2 hour 36 minutes. Postoperative course was uneventful. Patient was successfully initiated and advanced the diet while still on TPN during hospitalization, and was discharged on postoperative day 8.
