There is a $100 application fee for Associate Active Members. You may choose to send a check along with the support materials listed below, or you may pay by credit card via our secure web store. Please choose an option below:

Please remember that we will need you to email or fax the following documents in order to complete your application. If you fail to provide any of the following, your application will be considered incomplete and your induction into SAGES will be delayed:

  • A copy of your State Medical License
  • A copy of your surgical Board Certificate. Acceptable Boards
    include: American Board of Surgery, the American Board of Colon and Rectal
    Surgery or the American Board of Osteopathic Surgery (or international equivalent)

IMPORTANT: IF THE DOCUMENTS ABOVE ARE NOT IN ENGLISH, PLEASE PROVIDE A TRANSLATION OF EACH DOCUMENT.

  • A letter of recommendation from current Chief of Surgery, a colleague who is familiar with your surgical work, or from a previous instructor in endoscopy and/or laparoscopy.
  • A letter of recommendation from sponsoring SAGES Member. Membership List link available after payment of application fee.
