Reducing warm ischemic time can optimize islet cell yield in pancreatic islet cell autotransplantation following total pancreatectomy. Performing a total pancreatectomy without division of the pancreatic neck may also potentially increase islet cell yield. This video demonstrates a robotic total pancreatectomy with autoislet transplantation in a 20 year old patient with chronic pancreatitis secondary to a SPINK mutation. The robotic total pancreatectomy is achieved without division of the pancreatic neck and by maintaining vascular inflow (GDA and splenic artery) to the entire specimen right up until extraction. Autoislets are then infused with robotic assistance.
