We herein describe the repair of a right renal artery aneurysms with a Y-shaped authologous safenous graft, using the DaVinci Surgical System. This 41-years-old female accidentally discovered a saccular aneurysm of the right renal artery with a maximum diameter of 20 mm, with 1 in and 4 out.
The total operative time was 350 minutes and the estimated surgical blood loss was about 200 ml. Warm ischemia time was 58 for the posterior branch plus 24 minutes for the second declamping. The patient resumed a regular diet in postoperative day 2 and the hospital stay was 4 days. No intraoperative or postoperative morbidity was noted.
The experience of our group counts 5 other renal aneurysm repair performed with a robotic assisted technique. In this case the presence of five different arterial branches involved in the reconstruction makes this procedure extremely delicate. Finally, we confirm that robot-assisted laparoscopic repair of renal artery aneurysms represents a valid alternative to open surgery also in complex cases. A CT scan performed 2 months later shows the patency of all the reconstructed branches.
