This is the case of a 51 y/o female PMH gastric bypass whom presented to the ED with a one day history of acute abdominal pain. CT scan showed pneumoperitoneum and perforation of the gastric pouch as well as a gastro-gastric fistula. Gastro-gastric fistula only occur in a very small subset of gastric bypass patients and a perforation is very rare. The surgical management is both difficult and complex. Included is a video presentation highlighting the management and repair of such a case.
