Pancreaticojejunostomy (PJ) is the most critical step of pancreaticoduodenectomy (PD), as any leak from PJ can lead to a pancreatic fistula. Clinically relevant pancreatic fistula is the index of PD outcomes. These depend upon patient factors & surgeon apart from the technique. Ischaemia at the anastomotic suture line being close to duct margins is an important factor. Our technique avoids multiple such sutures through the duct margin, in an attempt to avoid ischaemia. The anastomosis is reinforced then by dunking in a standard manner. We present our innovation with successful experience of 13 cases.
