Esophageal reconstruction after esophagectomies for cancer can be challenging. The gastric pull-up procedure is the ?rst line procedure for esophageal reconstruction due to the rich vascular supply of the stomach, a functional mucosa, the need for only a single anastomosis and the preservation of the pylorus. The purpose of this report is to present a case of colonic interposition after graft necrosis status post Ivor Lewis for distal esophageal neoplasm and describe our approach.
