Large paraesophageal hernias in which the crura cannot be primarily approximated present a clinical challenge. The purpose of this video, is to demonstrate a left diaphragm relaxing incision as an alternative surgical technique for management of a type IV paresophageal hernia. A review of current management strategies is included, as well as anatomic landmarks necessary to perform the relaxing incision.
