We present the case of a 47 years old female status post open appendectomy for perforated appendicitis, with a resultant 8×7 cm incisional hernia. An alternative hernia repair is presented with the use of a mesh positioning system, a single tubing system connected to an inflatable balloon, which atttaches and secures the mesh to the anterior abdominal wall. The procedure is described and the result is shown. The patient recuperated very well and was discharged on postoperative day 1. The mesh positioning system is a promising alternative to the standard laparoscopic ventral hernia repair approach.
