Preoperative workup for revisional bariatric surgery includes an upper endoscopy to better delineate anatomy. On rare occasions, abnormal findings such as masses are also discovered. Our video presents a patient with a duodenal mass found on preoperative endoscopy for a patient with a previous Molina band, now being converted to a gastric bypass. Two techniques for managing this finding in the setting of a planned laparoscopic roux-en-Y gastric bypass include either resection of the remnant stomach with a partial duodectomy to incorporate the mass, or in this case, intraluminal resection of the mass via a Heineke-Mikulicz pyloroplasty.
