This is a video presentation concerning a patient undergoing laparoscopic resection of a 7cm multiloculated cystic lesion in the hepatic hilum. The patient had previously undergone open cholecystectomy 20 years prior. The lesion was found to be a dilated cystic duct remnant due impaction of stones at the insertion of the cystic duct to the common bile duct and was successfully dissected free from the hepatoduodenal ligament. The resultant defect in the bile duct was closed over a t-tube.
