This video abstract demonstrates the laparoscopic repair of a large congenital 7x6cm Morgagni hernia defect with incarcerated stomach, proximal duodenum, and transverse colon. The patient was symptomatic for prior obstructive symptoms, postprandial emesis and substernal chest pain, and severe GERD refractory to medical management. On entry, the incarcerated structures were first reduced. The Morgagni defect was then primarily repaired with #0 Surgilon mattress sutures buttressed with plegets. Although the repair was satisfactory, given its large size, we elected to reinforce this repair with mesh, secured in place with a double crown technique. Post operatively the patient did well with resolution of the preoperative symptoms
