Repair of abdominal wall hernias adjacent to bony structures are challenging. Open as well as laparoscopic approaches have been described with placement of prosthetic mesh. Laparoscopic repair provides anatomic detail, room for sufficient area of overlap beyond the defect, and the benefits of a minimally invasive approach. We present an obese patient with large left flank hernia containing small bowel and colon resulting from iliac bone graft harvest.
