This video illustrates the key technical components of a laparoscopic repair of a giant type IV paraesophageal hernia. Due to a wide hiatal defect, the crura could not be brought together initially and a right-sided crural relaxing incision was performed to reduce tension on the repair. A Collis gastroplasty was also performed using a wedge-fundectomy technique to address a short esophagus that persisted despite extensive esophageal mobilization.
