En bloc resection of the superior mesentericvein /portal vein with concomitant venous reconstruction is required in patients with locally advanced pancreatic cancer. However, it is technical challenge to perform laparoscopic pancreaticoduodenectomy with major vascular resection and reconstruction. Herein, we present a case of laparoscopic pancreaticoduodenectomy with major vascular resection and reconstruction using anterior ‘superior mesenteric artery first’ approach. The patient suffered from a pancreatic tumor which located at the head of pancreas. The tumor involved the superior mesenteric vein. The operative time was 462 min and the estimated blood loss was 200 ml. The time for venous reconstruction was 34 min. The patient discharged on the 11th postoperative day uneventfully.It is safe and feasible to perform laparoscopic pancreaticoduodenectomy with major vessels resection and reconstruction. The anterior ‘superior mesenteric artery first’ approach can facilitate this procedure, decrease the operative time and blood loss.
