This is a video of a laparoscopic Morgagni hernia repair in a 63 year old gentleman with severe pulmonary hypertension who had a symptomatic Morgagni hernia after motor vehicle trauma and explosion. We discuss perioperative management and the use of a valve-free insufflation system that enables a stable pneumoperitoneum, which allowed us to achieve operative exposure with minimal insufflation pressures (6-8 mmHg).
