Compression of the celiac artery by the median arcuate ligament (MAL) can cause graft dysfunction in patients with status-post liver transplant. Most reports describe this condition arising in the intra-operative or immediate post-operative period. We describe the case of a 21-year-old male who underwent laparoscopic MAL release for celiac artery compression 18 years after orthotopic liver transplant.
