We present the case of a 76 year old female with extensive past medical history who presented to our emergency department in hemorrhagic shock. CT scan of the abdomen revealed a left upper quadrant hematoma with hemoperitoneum, which prompted urgent exploration. On diagnostic laparoscopy, hemorrhagic ascites was encountered and then a large ruptured splenic hematoma was identified. Splenectomy was successfully completed and all bleeding controlled laparoscopically.
