In rectal surgery, traumatic injuries to presacral vessels are associated with high morbidity and adverse outcomes like excessive blood loss, transfusions and conversion to laparotomy. This video demonstrates a new approach for the management of a presacral hemorrhage during a laparoscopic low anterior resection (LAR). After initial control of the bleeding vessel with laparoscopic graspers, we used two subsequent Hemorrhage Occluder Pins through a 12 mm right lower quadrant trocar to achieve hemostasis and avoid conversion to laparotomy or other unfavorable outcomes.
