This is the presentation of a 54-year-old male who presented with a history of intermittent chronic abdominal pain. On imaging, he was found to have intestinal malrotation with small bowel wall thickening concerning for intermittent volvulus. Given his symptoms and findings on CT scan, he underwent a diagnostic laparoscopy and a Ladd's procedure was performed. The case is presented and the operation is narrated in this video.
