This video demonstrates a laparoscopic excision of a type I choledochal cyst in a 17 year old female patient. She presented with right upper quadrant pain and was subsequently found to have common bile duct dilation greater than 2 cm by MRCP. The operation is shown and explanation of each step is given. The distal extent of the cyst is located within the head of the pancreas and dissection of the common bile duct out of the head of the pancreas is shown. Reconstruction was performed in a single layer with a side to side choledochoduodenostomy using a running absorbable stitch. The video demonstrates advanced dissection techniques within the hilum of the liver and into the pancreas, as well as reconstruction to small bile ducts.
