We present a case of a laparoscopic conversion of a Toupet fundoplication to Roux-en-Y gastric bypass as a treatment option for recurrent hiatal hernia in a morbidly obese patient. Intraoperatively we found that the gastric pouch became ischemic after the take down of the fundoplication associated to an extensive and difficult dissection of the hiatus. We proceeded to resect the gastric pouch and perform a Roux-en-Y esophageal–jejunum anastomosis. The technique, the preoperative and postoperative management are described.
