With the demand for weight loss surgery increasing, surgeons are likely to encounter with increasing frequency patients desiring these operations who have had previous foregut surgery. This creates a "revisional surgery" situation, significantly increasing the technical difficulty of the procedure. Presented for your review is a technique I have developed with a series of 15 conversions first taking down the Nissen Fundoplication then performing a retrocolic, retrogastric Roux-en-Y gastric bypass.
