A 80-year-old male underwent an uncomplicated endovascular repair of abdominal aortic aneurysm (EVAR). The routine follow up imagery showed a persistent leak from the middle sacral artery and an increasing size aneurysm up to 6.6cm. This video describes the first known case of a laparoscopic ligation of the median sacral artery to manage a type II endoleak post EVAR.
