Innovating Training Networks for Interactive E-learning as the Method of Choice to Benefit from Big Data Analytics and Computing in the Remote Multidisciplinary Approach and Decision Making to Optimize Quality in Cancer Treatment.

Back-round:Crucial limitations in the current system of the cancer treatment have been defined and may be responsible for lack of accurate diagnosis or lack of mastery in the decision making about cancer treatment.

Objectives: Remote, interactive, collaborative Tele-medicine system (PROMETHEUS pn:2003016) and Hyper-computer (ARIS IBM Hyper-computer of the Hellenic Minstry of Education) assisted learning (E-Learning) in the context of innovating training networks using the Expert Leaded Medical Protocols (ELMPs in Oncology) may provide an infrastructure and method remote, systematic, integrated, interactive, holistic, multidisciplinary, specialized and personalized evaluation of surgical oncology cases between two or more remote hospitals by E-Learning sessions. Participants use interactively Telemedicine Systems (PROMETHEUS pn:2003016) for Tele-Radiological (TRE), Tele-Pathological (TPE), Tele-cytological and Tele-Microbiological (as well as genetical with the -omics) evaluation of oncology cases and manage the ELMPs in Oncology which are based on our digitalized Atlas for Cancer Staging and Handbook.

Material and methods: The proposed experimental interactive E-Learning Innovating Training Networks are based at least on two partners. The partners and the server are linked with each other via a network. In the server, images taken from Digital Cameras, Endoscopic Cameras, US, CT, MRI and/or PET Scanners are archived in the picture archiving and communication system (PACS). Thus, in each interactive E-Learning session participants from each center use basically our available and granted by the research program of ‘’Excellence’’ of the Hellenic State of Scholarships technology Tele-medicine based device (three PROMETHEUS pn:2003016), an internet and Cloud based network for tele-communication and archiving of data as well as our standard interactive e-Learning platform and the also granted IBM hyper-computer ARIS infrastructure from the Hellenic Ministry of Education. Participants learn interactively: i.The remote multidisciplinary approach to cancer patients for treatment and decision making, ii. The use of (TS) in Tele-radiology, Tele-pathology, Tele-cytology, Tele-microbiology, Tele-molecular biology for diagnosis, classification, decision support and making, iii. The use of cloud technology for projection, archive and management of digital text and medical images data, iv. The use of Big Data analytics and computing analysis and computation of the main lagging indicator (morbidity, mortality and quality of life indicators) and decision making on a global level.

Preliminary Simulating Results: With regard to the TRE integrated with the TPE of Liver (L), Pancreas (P) and Renal (R) lesions, PROMETHEUS pn:2003016, has been shown to be feasible and reliable for remote evaluation of neoplasms and for cancer treatment decision support and planning. The integrated analysis of reliability of TRE and TPE remote evaluation of the L, P and R showed respectively: (L): Sensitivity= 96.6%, Specificity=100% and Accuracy=97.5%. (P): Sensitivity= 93.3%, Specificity=100% and Accuracy=95.1%, (R): Sensitivity= 96.7%, Specificity=100% and Accuracy=97.6%.

Conclusion:The process to build a better safety culture, improve outcomes, take preventive measures, make evidenced decisions and enhance personalization in cancer treatment starts with a multidisciplinary approach, decision making and treatment and is completed with Big Data Analytics and Computing. This integrated technology and method seems optimum for interactive e-learning and for the quality of cancer treatment worldwide.
