This video demonstrates the laparoscopic management of a giant type IV paraesophageal hernia. The patient is a 35-year-old woman had a known paraesophageal hernia who presented to our clinic with worsening dysphagia, chest pain, nausea and emesis for the past year. Her pre-operative CT scan and upper gastrointestinal barium swallow are shown. She then underwent a laparoscopic repair for her paraesophageal hernia with a Toupet fundoplication. The key steps of initial hernia reduction, hiatal dissection, reduction and resection of the hernia sac, fundoplication and intra-operative upper endoscopy are shown and described.
