Revisional bariatric surgeries are challenge, especially when intraoperative anatomy is obscured by dense adhesions. Gastroscope plays an important role in defining the foregut anatomy. Here we present a case where despite difficult anatomy and extensive adhesions, the revision from vertical banded gastroplasty to roux en y gastric bypass was safely done laparoscopically with the help of gastroscope.
