We present a procedural video of what we believe is the first ever endoscopic guided gastrojejunostomy formation (EGG) in a patient that had previous Roux-en-Y gastric bypass surgery. The patient presented with severe malnutrition and gastric outlet obstruction from complete closure of the gastrojejunostomy secondary to fibrosis from recurrent marginal ulcer disease. Traditionally these patients have underwent gastric remnant feeding tube placement for nutritional support with eventual surgical revision of the gastrojejunostomy. We were able to successfully create a new gastrojejunostomy completely endoscopically and immediately restore gastrointestinal continuity.
