It Is Easy T Be o Proud of SAGES
t is easy to be proud of SAGES.We have the pride of excellence in leadership, the pride of excellence in innovation, the pride of superb inclusiveness and collegiality, and, as I noted in my last column, the pride of service. To this list we can add the pride of excellence, leadership, and innovation in surgical education. At every major surgery meeting and in all the hallways of leadership in American surgery, SAGES is recognized for our role in education. We started way back when, with our train the trainer’s program to make the dissemination of laparoscopy safer.We continued with the establishment of our premier surgical journal, Surgical Endoscopy.We wrote guidelines, we educated the legislators through the coding process, we educated hospital administrators about the value of investing in advanced equipment, we educated device companies about what was needed to make laparoscopy safer and better, and most of all, we educated and continue to educate other surgeons. We have permanently changed the face of surgical training by developing the high quality, validated Fundamentals of Laparoscopic Surgery (FLS) program.This didactic and hands-on program, meticulously developed over a decade, is now required by the American Board of Surgery of every surgical resident in the country who wishes to sit for their Boards. Taking the lessons learned from this valuable educational model, we have Fundamentals of Endoscopic Surgery (FES) soon to be launched. Recognizing a need in the arena of patient safety, Fundamentals for the Use of Safe Energy (FUSE) is now under development, and is poised to be the tool to educate surgeons everywhere about the safe application of the ever-proliferating energy sources in surgery.

You’ll also find SAGES Images Library, where you can find member-contributed images that you can use for education.A listing of our enduring materials, such as SAGES Pearls,Top 21, and Grand Rounds can also be found here. SAGES also contributed online materials to the national surgery curriculum via SCORE (the Surgical Council on Resident Education), and were recognized with membership in the elite group of surgical societies who have worked to develop that curriculum and teaching materials. SAGES, never satisfied to rest on our laurels, is rapidly developing the SAGES University.Watch for the launch of this member-directed product, which will include a journal club, MOC and CME tracking, and and self-assessment tools.This one-stop education product will allow members to keep up-to-date and organized, and will help us all fulfill American Board of Surgery Maintenance of Certification requirements using tools that are pertinent and meaningful to our practices. The gem in our crown of education, though, is our annual meeting.This is where you can meet the experts and those who work in the trenches beside you.You can ask questions and participate in discussions. You can visit the exhibit halls, take a hands-on course, and go relax at the Main Event with all your colleagues. Come join us. Teach and learn. Participate in SAGES. Natural Orifice Surgery Consortium for Assessment and Research® (NOSCAR®)
A joint initiative supported by the American Society for Gastrointestinal Endoscopy and the Society of American Gastrointestinal and Endoscopic Surgeons
The Natural Orifice Surgery Consortium for Assessment and Research® (NOSCAR®), a joint effort of the American Society for Gastrointestinal Endoscopy (ASGE) and the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES), is pleased to announce that institutions selected to participate in a multicenter human trial on transoral and transvaginal cholecystectomies (gallbladder removal) using Natural Orifice Translumenal Endoscopic Surgery® (NOTES®) are enrolling patients to take part in the study. Cholecystectomy, or gallbladder removal, is one of the most common surgeries in the U.S.; approximately 750,000 are performed annually. The NOSCAR study uses the mouth or vagina as routes to the gallbladder. Rather than making up to five incisions in the abdominal wall, tools are passed down the mouth and through a hole created in the stomach (transoral/transgastric) or through the vagina (transvaginal). The prospective multicenter trial compares NOTES cholecystectomy versus conventional laparoscopic cholecystectomy. NOSCAR has granted funds to selected institutions to conduct these trials. Institutions participating in the trial have demonstrated a strong commitment to developing the basic science that forms the foundation of NOTES and have already performed human NOTES cases under an Institutional Review Board protocol; approximately two hundred patients will be enrolled in the clinical trial. New For 2011: American Board of Surgery (ABS) Maintenance of Certification (MOC) Part 2 SelfAssessment Credit Available at the 2011 Annual Meeting
The SAGES Continuing Education Committee’s Assessment Task Force continues to review SAGES’ evaluation methods for the Annual Meeting for areas of further improvement to help ensure that SAGES is meeting its overall educational objectives. Completing the survey will be mandatory in order to claim CME credit for the 2011 meeting. New for this year: Participants who complete the online survey and CME credit form who also participate in an online follow-up assessment in July are eligible to convert all of the CME credits claimed for participation at the Annual Meeting to Self-Assessment MOC Part 2 credits. Your feedback is vital to enhancing the program in years to come.Thank you in advance for completing your Annual Meeting surveys.

FES Committee Update
The FES Task Force, headed by Jeffrey Marks, MD and John Mellinger, MD, is in the final stages of the development and validation.of the FES educational program. Dr. Mellinger will co-chair a panel presentation at the SAGES Annual Meeting on Wednesday, March 30, entitled “Incorporating FLS and FES into Your Residency”. Surgical program directors, residency coordinators, and simulation and skills lab staff are encouraged to attend The upcoming San Antonio meeting will also offer attendees a “sneak peek” at the newly-completed online didactic curriculum at the SAGES booth, plus, in the Learning Center there will be an FES station offering hands-on experience with the FES skills modules. The cognitive multiple-choice, computer administered test has now been validated, using data from 350 residents, fellows, attending surgeons and gastroenterologists who took part in the pilot testing process.The data analysis and final review culminated with a ‘standard setting’ retreat attended by 11 SAGES members. Membership International Proctoring Course (IPC)
Pingchang, China From January 17 to 20, 2011, Mark Pleatman and I participated as volunteer faculty for a laparoscopic cholecystectomy course at the People’s Hospital in Pingchang City, Sichuan Province, China. This course, sponsored by the SAGES Global Affairs Committee, was the second such course conducted at this location.We trained two young surgeons over the course of three days, and in all proctored 12 cases, with each trainee acting as either surgeon or assistant in each case. The trainees advanced quickly and the course was extremely well received by the surgeons, the OR staff, department leadership, and hospital administration. We were able to follow-up with a surgeon trainee from the 2009 SAGES course in Pingchang, who reported that he had performed over 100 laparoscopic cholecystectomies since his participation in the course. The surgical landscape in China is a study in contrasts.While laparoscopic cholecystectomy is still novel in Pingchang, 6 hours away in a 3000 bed hospital in Chongqing, Chinese laparoscopic surgeons perform a very high volume of advanced laparoscopic hepatobiliary procedures. Being fluent in Mandarin Chinese, I had occasion to converse at length with several local, regional, and national leaders in laparoscopic surgery in China, and can report that our Chinese colleagues are extremely eager to build on their collaborative relationship with SAGES.

Research and Career Development Career Development Workshop
Saturday, April 2, 2011, San Antonio,TX The SAGES Research and Career Development Committee will be hosting its 3rd Annual Career Development Workshop. The workshop is now scheduled to take place Saturday, April 2, 2011 during the SAGES Annual meeting in San Antonio. This course has been designed by the SAGES Research Committee in order to provide its membership with education and personalized training on obtaining the skill set required for academic success. The goal of this full day program is to provide fellows and junior faculty with a venue for interaction with experienced faculty with at rack record of successful publication, funding, and mentorship.This experience will serve to “set the bar” for a formalized career development effort at the participants home institution.Topics will range from developing and organizing research studies to successful grant and manuscript writing strategies. Each participant will bring the specific aims portion of a research grant that will be edited, revised, and improved for a submission to SAGES and/or other funding sources. Small group discussions on such protocols will provide practical training in research methodology, proposal writing and manuscript preparation. Resident Education Committee Resident Courses
The Resident Education Committee has tentatively scheduled 10 resident courses for 2011. Invitations are sent to residency program directors approximately three months prior to the course. Annual Meeting to Feature Five Innovative Hands-On Courses
MIS Colorectal Surgery Hands-On Course
This half-day practical cadaver lab course is designed for general and colorectal surgeons, fourth year or chief residents and MIS or colorectal fellows. All applicants should be familiar with advanced laparoscopic techniques and wish to expand their skills in laparoscopic colon and rectal surgery. Techniques for straight lapaorscopic and single incision colectomies, bowel mobilizations, and vessel division will be taught with an emphasis on oncologic principles.The course will emphasize common alternative approaches including lateral-to-medial, medial-to-lateral, and hand-assisted techniques, to facilitate resection of the entire intra-abdominal colon and the rectum. Lab stations will have a 1:3 faculty: participant ratio.
Minimizing MIS Hands-On Course
This course will consist of a hands-on animate lab in which participants will receive instruction by experts in techniques to minimize the invasiveness of their MIS approach. Strategies for downsizing ports, reducing the number of ports, and using 3mm and 2mm instrumentation will be employed. Participants will have the opportunity to use a variety of novel instrumentation, including percutaneous minlaparoscopic instruments as well as intracorporeal retractors. Additionally, single incision techniques for laparoscopic surgery will be demonstrated in combination with minlaparoscopic instruments to afford practice with both types of strategies. Single incision techniques will include the use of multiport access devices, suitable camera systems, and articulating instruments. Lab stations will have a 1:2 faculty to participant ratio.
Endoscopy for Surgeons Hands-On Course
This hands-on course will complement the didactic portion of endoscopy for surgeons providing exposure to various techniques in an explant model. Participants will have exposure to therapeutic endoscopic techniques including polypectomy, tattooing, management of GI bleeding, stenting, EMR, mucosal ablation for Barrett’s, and endoscopic GERD therapy.
Bariatric Surgery Hands-On Course
The Hands-On portion of the Bariatric Surgery Postgraduate Course will focus on the technical aspects of laparoscopic sleeve gastrectomy, gastric banding and hiatal hernia repair. Participants will also be able to do single incision laparoscopic approaches for gastric banding and sleeve gastrectomy. The Fellowship Council
Fellowship Council Website Now Open for New Member Applications
The Fellowship Council was created to foster the development of high quality Fellowships in MIS, GI, Flexible Endoscopy, Bariatric, Hepato-Pancreato-Biliary, Thoracic and Colorectal surgery. Stringent guidelines were created for the optimal education experience and implemented an accreditation process to ensure that our programs live up to these high aspirations. In 2004 the FC Match was instituted to facilitate the application process and to level the playing field for programs and applicants. The Fellowship Council continues to grow both in numbers and in influence. Currently representing over 150 programs, the Council represents almost 90% of the postgraduate training programs in Advanced GI surgery in North America. The Council’s influence is reflected in its close ties with leading surgical specialty societies including: AHPBA, ASCRS, ASMBS, SAGES and SSAT. The Fellowship Council is accepting new member applications February through April 2011.
