SAGES Flexible Endoscopic Surgery Course for MIS Fellows Registration

Columbus, Ohio November 12-13, 2009 at the Ohio State University.

Registration: This course is open only to 1st or 2nd year fellows. Registration is limited to the first 30 registrants. Priority registration will be given to SAGES Candidate Members. You may register only one fellow per program for any scheduled course. There is no fee. Participants are responsible for their transportation from the airport to the hotel.

Hotel/meals: Hotel rooms will be provided for the nights of Wednesday, November 11th and Thursday, November 12th. Fellows will be sharing a room with one other fellow. Rooms will be assigned on an ad hoc basis. If a room is required for Friday night or Saturday night, it will be provided at the participant's expense. Breakfast, lunch & dinner on Thursday, and breakfast & lunch on Friday, are included in the program.

Transportation: SAGES will provide shuttle transportation to Port Columbus International Airport (CMH) at 1:30PM on Friday, November 21st. (Note: All early departure transportations will be the responsibility of the individual fellow)
