Course Description:

Minimally invasive surgery is not a static phenomenon. It continues to evolve as new technology is introduced, new procedures are developed and new approaches for standard procedures are created. Surgeons in practice and in training need hands on experience to determine which new developments are relevant for them and to master new technology and techniques. This hands on course will discuss new developments in MIS and will give attendees a chance to learn new skills in an animate laboratory setting. A sampling of the latest equipment, approaches and techniques will be available under the instruction of laparoscopic experts.

Objectives:

Upon completion of this course, the participant will:

  1. Recognize and understand the advantages and limitation of new instrumentation for minimally invasive surgery.

  2. Identify new approaches for gaining access to the abdomen and endoscopic surgery, including their indications and techniques.

  3. Become familiar with some of the newest minimally invasive procedures including surgery on the thyroid gland, parathyroid, esophagus, liver and spleen.

Laboratory & Lunch

The lectures will be followed by a four hour hands-on laboratory. There will also be a lunch for lab participants. The participants will perform suturing on inanimate models, as well as kidney and/or adrenal resection and enteric anastamosis on porcine animate models. Expert faculty will instruct participants in a 1:3 faculty:trainee ratio.

Hands-On Inanimate Lab Location: Barnes Jewish Hospital/Washington University Lab Facility

Objectives:

Upon completion of this course, the participant will have been presented with the following information, and should have an understanding of the issues regarding laparoscopic common bile duct exploration.

  1. Options for making the preoperative diagnosis of common bile duct pathology, including laboratory tests and imaging studies.

  2. Options for imaging the bile ducts intraoperatively, including cholangiography and ultrasonography.

  3. Techniques for intraoperative cholangiography.

  4. Indications for laparoscopic common bile duct exploration.

  5. Contraindications to laparoscopic common bile duct exploration.

  6. Access routes for common bile duct exploration, including transcystic and transductal approaches.

  7. Techniques of laparoscopic common bile duct exploration, including balloon, basketing, lithotripsy, and endoscopic methods.

  8. Results of laparoscopic common bile duct exploration.

  9. The role of ERCP +/- S. (endoscopic retrograde cholangiopancreatography +/- sphincterotomy. )

  10. An overall algorithm for the management of common bile duct pathology.

Laboratory

The lab will feature instruction in cholechoscopy and choledochotomy technique and instrumentation, including basketing lithotripsy and CBD suturing. Participants will also practice cholangiography techniques and fluoroscopic basketing. CBD experts will provide tips in avoiding complications and successfully managing complex cases.
