The majority of surgically treated rectal cancer via a transanal approach has been performed outside of the United States. This video abstract serves as a framework for the delineation of a potential training pathway for transanal total mesorectal excision (taTME) for rectal cancer. Appropriate training in this emerging technique is essential in order to ensure patient safety and high quality oncologic outcomes.
