Single port surgery through the umbilicus has been proposed as an embryologic natural orifice surgical approach. For colorectal resection concerns regarding visualization and retraction have been raised when considering the feasibility of single port colon surgery. We present in this video the treatment of a sigmoid volvulus, via an umbilical single port laparoscopic sigmoidectomy, in an ideal candidate. Challenges and approaches to overcome them in a single port colectomy are shown. We attempt to demonstrate that the procedure can be performed with excellent visualization and retraction for sigmoid colectomy, appearing substantially similar to a standard laparoscopic left colectomy.
