This video shows an emergent laparoscopic repair of a type IV paraesophageal hernia, which acutely volvulized, that contained the antrum and duodenum. Initial attempt at endoscopic decompression and PEG gastropexy images, axial CT imaging, barium upper GI esophagram, and video of the laparoscopic repair for the paraesophageal hernia are all depicted. The repair includes a relaxing incision, absorbable mesh placement, and a Toupet fundoplication.
